The SRT Intake Form is used only by SRT volunteers and only when the Directors of SRT have agreed to bring a dog into our rescue organization.  Please complete the form with as much detail as is available.  The form will automatically be sent to the SRT Directors.

  • Animal Intake Information

    Please complete all information in this section to the best of your availability based on what you know of the dog to-date.
  • Please 'other', please specify.
  • Please specify the age. If you don't know, please use an approximation (e.g. 2-4 years). Specifics are better!
  • Please specify the weight. If you don't know, please use an approximation (e.g. 10-15 pounds). Specifics are better!
  • Provide microchip number if animal is chipped
  • Provide the date that the animal was picked up (from shelter, from surrenderer, etc.

  • Provide name of shelter where animal was picked up or the name of the owner or stray surrenderer.
  • Provide indicate where the animal has been dropped off (Almost Home, Foster, etc)
  • Please provide a listing of vaccinations received.
  • Has the animal been spayed / neutered?
  • Does the animal appear to be house trained?
  • Does the animal appear to be crate trained?
  • Does the animal appear to be leash trained?
  • Describe the animal's behavior with children if you have observed such.
  • Describe the animal's behavior with / around other dogs if you have observed such.
  • Describe the animal's behavior at the groomer if you have observed such.
  • Describe the animal's behavior at the veterinarian if you have observed such.
  • Describe the animal's personality - playful? shy? timid? alpha? submissive? unique / cute characteristics? etc?
  • Please attach a photograph (full body please) of the dog. Upload additional photographs in subsequent section.
