SRT Stray Surrender Form

The Stray Surrender Form is to be used by those who have found a stray Schnauzer or Schnauzer mix.  It is not to be used by those who own, and wish to surrender, a Schnauzer or Schnauzer mix (see Owner Surrender Form for this purpose).  Completion and submission of the form does NOT guarantee that SRT will take the animal into rescue.  Whether SRT takes an animal into rescue is at the sole discretion of the SRT Directors.  Please complete the form with as much detail as is available.  The form will automatically be sent to the SRT Directors.  Note:  We prefer that you have the dog scanned for a microchip before completing this form.  There is no charge for this service at veterinarians and retail pet stores such as PetSmart.

  • Animal Information

    Please complete all information in this section to the best of your availability based on what you know of the dog to-date.
  • Please 'other', please specify.
  • Please indicate the dog's age. If you do not know, please use an approximation (e.g. 0-4 years). Specifics are preferred!
  • Please indicate the dog's weight. If you do not know, please use an approximation (e.g. 10-15 pounds). Specifics are preferred!
  • There is no charge to have a stray scanned for a microchip at a local vet's office or at retail pet stores like PetSmart. If possible, please have animal scanned for a chip.
  • Has the animal been spayed / neutered?
  • Please describe where animal was found (address or street name (with cross streets) or any other pertinent information regarding location.
  • Does the animal appear to be house trained?
  • Does the animal appear to be crate trained?
  • Does the animal appear to be leash trained?
  • Any evidence that animal has bitten anyone or has the potential to bite?
  • Describe the animal's behavior with children if you have observed such.
  • Describe the animal's behavior with / around other dogs if you have observed such.
  • Describe the animal's behavior at the groomer if you have observed such.
  • Describe the animal's behavior at the veterinarian if you have observed such.
  • Describe the animal's personality - playful? shy? timid? etc?
  • Please scan / submit vet records if you have taken the dog to a veterinarian.

          • Statement Of Understanding

            This section must be read and agreed to by the person who is completing this form. A Stray Surrender Form cannot be submitted without agreeing to the Statement of Understanding.
          • Schnauzer Rescue of Texas (SRT) is not obligated to take the animal submitted on this form simply because the form has been completed / submitted. Acceptance of this animal is at the sole discretion of the SRT Directors. SRT will contact you promptly regarding your request and our decision. As the finder and/or relinquisher of this dog, you acknowledge that by signing this form - by clicking the 'I Agree' button above, that you have no ownership interest in this dog. If you own this dog, you should complete the Owner Surrender Form instead.
